Client Face Sheet: Printable Demographics
The Client Face Sheet gives you a printable demographics report in PDF form for a particular client.
Among other things this includes a clients:
- name
- DOB
- address
- phone numbers & email addresses
- and both primary & secondary insurance information

Access the Client Face Sheet
Access the client profile on any screen by going to:
- (left menu)
- On Mobile? To save space, we moved that menu to the bottom of the screen
- Click on the client
- Find the right menu, click the top button
- On Mobile? First click the top right button
- We then load a new tab in your browser with the client Face Sheet PDF:
Click the button and nothing happened? You likely have pop ups blocked. Good news it's an easy fix, check out our article on how to enable pop-ups for just our website!
- Use the in-browser controls at the top to either:
- Save as a PDF locally to your device

spa Note: Want to include your own logo on your statements and other areas in the system? By default, we don't include one unless you have a Practice Banner / Logo set.
Information Included in the Client Face Sheet
The following is included (if entered):
- Legal name
- Account #
- Date of Birth
- Gender
- Marital Status
- Employer
- Address, Phone Number, and Email
- Primary Insurance Information
- Secondary Insurance Information
