Adding Addendums On Locked Chart Notes

Did you or another provider in your group make a chart note in the wrong client account?

Did you make a mistake on a specific portion of the chart note that you locked and want to clarify?

Addendum Information

Once a chart note is finalized and locked, it cannot be modified or removed. This is common practice for medical records: an entry cannot be removed once signed (and locking is the electronic equivalent of signing) per our understanding of HIPAA.

Adding an addendum does not remove any of the original chart note, but both the original chart note and addendum will be visible from both the Records tab of a client under the specific appointment that an addendum was added to, as well as when printing records.

To add an addendum, you have two options:

1. Appointment Pop-up from the Schedule

To access the addendum this way, perform the following steps:

  • Click on your Schedule
  • Click on the appointment in question
  • Then click the far right button Add Addendum, shown below:
  • Once you complete the addendum, click the Save button to apply the addendum to that specific chart note.

2. From the Client's Records Tab

To access the addendum this way, perform the following steps:

  • Click the left menu Clients
  • Click on the client's name
  • Click on the 'Records' top tab
  • Find the appointment (chronological order) on the page

Note: If the appointment is 'collapsed' or hidden, click on the line item that starts with "Chart Entry..." shown below:

  • Then click the blue Add Addendum button shown below:
  • Once you have typed out your Addendum, click the bottom blue button Finalize & Lock to save
